Important: Form 8962 is faxed only when the IRS requests it, typically via Letter 12C. The fax number is on the letter. Without an IRS letter, attach Form 8962 to your Form 1040 and file together.
What is Form 8962?
Form 8962 (Premium Tax Credit) reconciles the Advance Premium Tax Credit (APTC) you received during the year against the actual Premium Tax Credit (PTC) you qualified for based on your final income. Anyone who:
- Enrolled in a Marketplace (Healthcare.gov or state exchange) plan
- Received APTC to lower monthly premiums
- Or wants to claim PTC at tax time
...must file Form 8962 with their tax return. If you forget to attach it, the IRS holds your refund and sends Letter 12C asking you to complete and submit it.
Why you got Letter 12C
Letter 12C is the IRS notice that says: "Your tax return is missing required information." For Form 8962 specifically, this means:
- Your Form 1095-A (Health Insurance Marketplace Statement) shows you received APTC
- But your tax return did not include Form 8962 to reconcile
- The IRS is holding your refund until you submit Form 8962
Letter 12C usually gives a 20-day response window. The fax number for response is in the letter, typically a regional service center fax line.

How to fill out Form 8962
You need your Form 1095-A from the Marketplace. Form 8962 has 5 parts:
- Part I: Annual and monthly contribution amount based on income
- Part II: Premium Tax Credit Claim and Reconciliation
- Part III: Repayment of Excess Advance Payment of the Premium Tax Credit (if APTC was more than your eligible PTC)
- Part IV: Allocation of Policy Amounts (only if shared with another tax family)
- Part V: Alternative Calculation for Year of Marriage
Most filers complete only Parts I, II, and possibly III.

What happens after you fax
The IRS resumes processing your tax return within 4-8 weeks. If your reconciliation shows you owe additional PTC repayment, the IRS adjusts your refund accordingly. If you over-paid, your refund is increased.
If you do NOT respond to Letter 12C within the deadline, the IRS may:
- Disallow your PTC entirely
- Send the case to collections for excess APTC repayment
- Apply the original refund to next year's tax obligation
Common mistakes
Faxing without the cover sheet referencing Letter 12C. The IRS uses Letter 12C ID to route your fax to the right examiner. Without it, your fax sits in a queue.
Wrong Form 1095-A data. Use the latest version of Form 1095-A, the Marketplace sometimes issues corrected versions (1095-A Corrected).
Missing income data. Your AGI on Form 1040 must match what you put on Form 8962 Line 2b.
Wrong tax family size. Include yourself, spouse, and all dependents, even those without health coverage.
