Why a cover sheet matters for IRS faxes
The IRS does not require a fax cover sheet, but every successful filing has one. Here's why:
- Routing accuracy: The cover sheet tells the IRS service center which form is enclosed. Without one, mail-room staff guess.
- Page count verification: The IRS counts received pages against your stated count to detect missing pages.
- Contact info: If anything is unclear, the examiner uses your phone number to call.
- Timestamp / case ID: When responding to letters (CP2000, audit), the cover sheet ties your response to the right case.
Required fields
Every IRS fax cover sheet should include:
- To: Internal Revenue Service + service center name (e.g., "Cincinnati EIN Operation", "Memphis CAF")

Common mistakes
Forgetting the cover sheet entirely. Some filers fax just the form. The IRS still processes it but case routing is slower.
Wrong page count. If your cover sheet says 5 pages but you fax 3, the IRS may suspect a transmission error and request retransmission.
Missing case reference for letter responses. Without the IRS letter number, your response sits in a queue while staff figure out the case.
Generic "tax filing" subject. Specify the form. "Form SS-4 EIN Application" routes correctly; "tax filing" requires manual review.
