VA Form 21-0781, Statement in Support of Claimed Mental Health Disorder(s) Due to an In-Service Traumatic Event(s), helps VA find evidence for a mental health claim such as PTSD, depression, or anxiety. The current March 2024 version covers combat events, personal events including military sexual trauma (MST), and other events like accidents, all on one form. It covers MST and personal assault on the same form, which used to need the 21-0781a.

The form is optional, and you skip it if you answered the mental health questions in the online 21-526EZ. You send it with a 21-526EZ for a new claim or a 20-0995 for a supplemental claim, or alone when VA asks by letter.

Most people complete Sections I and II, sign Section VII, and add Sections III and IV if they apply. Approximate dates are fine, and VA won't contact people you name. Inside the US, fax the signed pages to VA's disability claim fax, 844-531-7818, or mail them to the Evidence Intake Center in Janesville, Wisconsin. Confirm the number on VA.gov first, keep your fax confirmation, and check your claim status afterward.

What Is VA Form 21-0781 and When Do You Need It?

VA Form 21-0781 is the Statement in Support of Claimed Mental Health Disorder(s) Due to an In-Service Traumatic Event(s). You use it to tell VA what happened during service, so VA can look for records that back up a mental health claim.

It covers PTSD, depression, anxiety, bipolar disorder, and other conditions. The traumatic event can fall into three groups. Combat events include fighting the enemy, fear of hostile or terrorist activity, or serving as a drone crew member. Personal events include military sexual trauma (MST), physical assault, robbery, stalking, and partner abuse. Other events include car accidents, natural disasters, and witnessing a death not caused by the enemy.

The current version is dated March 2024 and replaces the June 2021 edition. It now handles MST and personal assault on the same form, so you check the event type in Item 8 instead of picking a separate form. If someone sends you an older copy, download a fresh one from the VA Form 21-0781 page on VA.gov.

The form is optional. VA's own PTSD claim page still tells you to fill it out when you file, because the details help VA find evidence.

You don't need a separate 21-0781 if you file online and answer the optional mental health questions in the 21-526EZ. The PDF is still worth having if you filed on paper, skipped those questions online, or got a VA letter asking for it.

How to Fill Out VA Form 21-0781, Section by Section

VA Form 21-0781 has ten sections, but most veterans fill in only five or six. Pages 1 and 2 are instructions. The fields VA reads are on pages 3 through 7.

SectionWhat it asksWho fills it in
I. IdentificationName, SSN, VA file number, birth date, service number, phoneEveryone
II. Traumatic eventsEvent type (Item 8), plus a short description, location, and date (9A to 9C)Everyone
III. Additional informationBehavior changes (10), official reports (11), other evidence sources (12)Mainly personal events
IV. TreatmentWhere and when you got care (13A to 13E)If you had treatment
V. RemarksExtra space (14)Optional
VI. VHA notificationMST consent choice (15)Only if you checked MST
VII. SignatureYour signature and date (16A, 16B)Everyone
VIII to XWitnesses, alternate signer, or representativeOnly in special cases

In Section II, keep each event brief. The form's own examples are short: "Mugged," "Back alley in Big Town, USA," "June 2007." Approximate dates like "Summer of '70" are fine. VA says it will still review your claim if you can't recall exact places or dates.

You can name people who were there, but you don't have to. The form states that VA won't contact anyone you name.

In Section III, Item 10 lists behavior changes such as changes in how much leave you took, panic attacks, alcohol or drug use, or a breakup. If your event was combat only, the form lets you skip to Item 11. Item 11 asks about official reports, including restricted or unrestricted MST reports, an option that began in 2005.

In Section IV, you don't need treatment dates for VA or military facilities if care started in 2005 or later. For private doctors or Vet Centers, VA needs VA Form 21-4142 and 21-4142a to request records.

Sign Section VII in ink. If you can only sign with an "X," two witnesses must sign Section VIII.

How to Fax VA Form 21-0781 to the VA

Fax VA Form 21-0781 to 844-531-7818 if you're in the US, or 248-524-4260 if you're abroad. Those are the disability claim fax numbers on VA's how to file a claim page, checked against VA.gov on October 26, 2026.

The 21-0781 itself only prints a mailing address: Department of Veterans Affairs, Evidence Intake Center, P.O. Box 4444, Janesville, WI 53547-4444. VA's Supplemental Claim form 20-0995 lists (844) 531-7818 as the Compensation Intake Center fax. Numbers can change, so check the current form or call VA at 800-698-2411 before you send.

Follow these steps:

  1. Download the current PDF and confirm the footer reads "VA FORM 21-0781, MAR 2024."
  2. Fill it in on a computer or by hand in black ink, one letter per box.
  3. Sign and date Section VII. Without your signature, the statement isn't certified.
  4. Scan every filled page in order, flat and well lit, so small print stays readable.
  5. Add a cover page with your name, VA file number or claim number, "VA Form 21-0781," and the total page count.
  6. Send it with your 21-526EZ or 20-0995, or alone if VA asked for it by letter.
  7. Save the fax confirmation showing the date, number, and pages sent.

Keep your cover page plain. Don't write your diagnosis, the event, or "MST" on it. Our guide to faxing the VA lists the other VA fax lines, such as Board Appeals and pension. If you need a format to copy, the fax cover sheet generator builds a clean one.

Mistakes That Slow Down a 21-0781

The biggest delays come from old versions, missing signatures, and unreadable scans. Each one can lead to a VA letter asking you to resend.

The most common slip is an outdated form. Copies of the June 2021 version still circulate online, and the March 2024 edition supersedes it, so pull the PDF from VA.gov each time. That fresh copy also covers MST and personal assault, which used to need the separate 21-0781a.

Section VII trips people up next. If someone else signs for you, they need the right paperwork on file before VA will accept it. An alternate signer needs VA Form 21-0972, and a representative needs a 21-22 or 21-22a in place first.

Writing too much in the boxes. Short phrases work better. Move longer explanations to Section V, Remarks, or attach extra pages. Put your name and VA file number on every added page so it stays matched to your file.

Faxing a blurry phone photo. Fax converts pages to black and white at low resolution. Light pencil, shadows, and tilted shots often turn unreadable. A proper scan with a document scanner app fixes this, and our guide on how to scan and fax from iPhone shows the steps.

Another slowdown is forgetting the claim form. The 21-0781 supports a claim; it doesn't start one. Send it with a 21-526EZ for a new or increased claim, or a 20-0995 for a supplemental claim.

A related mix-up is handing the wrong form to friends or family. Witnesses who saw the event or your behavior changes use VA Form 21-10210, Lay/Witness Statement, not the 21-0781. If you're sending several disability forms at once, the disability forms faxing guide covers how to keep each set in order.

How to Confirm the VA Got Your Fax

Your fax confirmation proves you sent it; your VA claim status shows VA filed it. You want both.

Start with the transmission report. It should show the VA fax number, the date and time, and the number of pages. Match the page count to your cover page. If one page is missing, resend the full set, not just the lost page. Our explainer on what a fax confirmation shows breaks down each line.

Next, check your claim on VA.gov or call 800-698-2411. Faxed documents may not show up right away. Give it time before you assume the fax was lost. VA doesn't publish a fixed posting time for faxed evidence, so don't count on a set number of days.

Keep a copy of everything you sent. Save the signed form, the cover page, and the confirmation in one folder. If VA later says a page is missing, you'll have proof of what went out and when.

Working with an accredited Veterans Service Officer? Send them a copy too. They can see your file and spot missing evidence early. VA notes that accredited VSO representatives don't charge for help with your claim, while accredited attorneys and claims agents can charge fees.

Should You Fax, Mail, or Upload Your 21-0781?

Uploading through VA.gov is the most direct route. Faxing works well if you filed on paper or can't use an online account. Mail takes the longest because of transit time.

MethodBest forWatch out for
Online at VA.govFiling a new claim with the mental health questionsNeeds a VA.gov account sign-in
Fax 844-531-7818Paper filers, or sending a form VA asked forBlurry scans and missing pages
Mail to JanesvillePeople without a scanner or phone cameraTransit time and no instant receipt
In personGetting help at a VA regional officeTravel and office hours

Faxing gives you a dated transmission record within minutes of sending. That record matters if you're close to a VA deadline.

A fax goes straight to VA's intake line, with no envelope in transit. Treat it carefully anyway. Double-check the number before you press send, because a wrong digit sends your statement to a stranger.

The guide to faxing the VA from iPhone walks through the phone side if you don't own a fax machine.

If this form brings up hard memories, it's fine to stop and come back later. The Veterans Crisis Line is open around the clock: dial 988 and press 1, or text 838255.

Faxing Your 21-0781 With Faxend

Faxend lets you send VA Form 21-0781 from your iPhone, Android phone, or any browser at faxend.com, with no fax machine or landline. You can scan the signed pages with your phone camera, add photos, or upload a PDF.

Faxend can add a cover page with the recipient, sender, subject, and a short message, and it counts as one page. It also tracks each fax and tells you by email or phone notification when it's delivered or if it fails. If a fax fails to send, Faxend automatically returns the pages or credits used for that fax.

It has limits worth knowing. Faxend sends PDFs and images, so convert other files first. It doesn't check your form for errors, and it can't tell you whether VA has added the pages to your claim. Use VA.gov or VA's phone line for that.

To send now, open faxend.com/send in any browser or compare Faxend plans and page packs. On your phone, get the https://apps.apple.com/app/apple-store/id6761759879?pt=128215051&ct=fxb-fax-va-form-21-0781&mt=8 and scan the pages in order.
