What to put on a fax cover sheet
- Recipient: name, department or office, and fax number.
- Sender: your name and a phone or fax number where you can be reached.
- Date and page count: the total number of pages including the cover sheet, so the recipient can tell if anything is missing.
- Subject and a short message: what the documents are and what you need the recipient to do.
- Confidentiality notice: for medical, legal, financial or personal information.
Which template to use
- Standard: everyday business and personal faxes, such as contracts, applications or forms.
- Confidential: adds a notice asking anyone who receives the fax by mistake to notify you and destroy it.
