How to Create a Fax Cover Sheet from iPhone

A fax cover sheet takes about two minutes to create on an iPhone, and it can save your recipient a lot of confusion about who sent the fax and why.

What is a fax cover sheet?

A fax cover sheet is the first page of a fax transmission. It tells the recipient who sent the document, who it is addressed to, and what the pages that follow contain.

Think of it as the envelope for a physical letter. The content travels inside. The cover sheet identifies the package.

Cover sheets became standard in the 1980s when shared fax machines sat in open offices. Someone needed to know which pile of paper belonged to them. The habit stuck, and in regulated industries it became a compliance requirement.

Today, most faxes go directly to a dedicated number or an email inbox. A cover sheet is still useful, but it is not always mandatory. More on that later in this guide.

What to include on a cover sheet

A good cover sheet is short. One page is always enough. Here are the fields worth including.

  • To: recipient's full name and organisation
  • Fax number: the number you are sending to
  • From: your full name and contact details
  • Date: the date of transmission
  • Subject or Re: a one-line description of the content
  • Page count: total pages including the cover sheet
  • Notes or message: optional, 2–4 sentences maximum

For medical or legal faxes, add a confidentiality notice at the bottom. A standard notice reads something like: "This fax may contain confidential information. If you received it in error, please destroy it and notify the sender."

The HHS HIPAA Privacy Rule does not dictate exact cover sheet language, but a confidentiality statement is considered a reasonable safeguard under the minimum necessary standard.

How to create one on iPhone

You have several solid options. None require a desktop computer.

Option 1: Apple Pages

Pages is free on every iPhone. Open it, tap the plus icon, and search for "letter" or "report" templates. Pick a clean one-column layout. Replace the placeholder text with your cover sheet fields. Export as PDF when done.

Pages exports clean, well-formatted PDFs that fax machines read without issues.

Option 2: Google Docs

If you already use Google Workspace, open the Docs app, create a blank document, and type your cover sheet. Tap the three-dot menu, choose "Share and export," then "Send a copy" as PDF. Save it to your Files app.

Option 3: A notes-to-PDF shortcut

Open the Notes app. Type your cover sheet content. Tap the share icon, then "Print." On the print preview screen, pinch outward on the preview thumbnail. This converts the note to a PDF. Tap the share icon again and save to Files.

Its a lesser-known trick but it works surprisingly well for simple cover sheets.

Option 4: Download a template

Search "fax cover sheet template PDF" in Safari. The standard fax format has not changed in decades, so any generic template will work. Download it to Files, open it in Markup, and fill in the fields by hand using the text tool. Save when done.

Cover sheets for HIPAA and legal faxes
