What a fax cover sheet template must include

A fax cover sheet needs six things. Recipient name, recipient fax number, sender name, sender phone, date, and total page count. Everything else is optional. The page count matters most. It lets the receiving office confirm that nothing was lost in transmission.

A subject line helps a busy front desk route the fax. A short message tells the reader what to do with the pages. Two lines are enough. Long messages belong in the document itself, not on the cover.

Confidential and medical faxes need one more block. A notice at the bottom tells anyone who receives the fax by mistake to call you and destroy it. HIPAA does not require a specific wording, but the U.S. Department of Health and Human Services expects reasonable safeguards when protected health information is sent by fax. Medical offices fax records and referrals every day. Prior authorization forms go the same way. The patient name and date of birth belong on the cover. The personal template is for the one-off fax to a landlord or a school. It also suits a family member. It skips the company fields and the legal notice. Nobody needs a confidentiality clause on a signed lease renewal.

Government forms are a special case. The IRS accepts faxed forms such as Form 2848 and Form SS-4. That cover sheet should carry the form name and your taxpayer ID. There is a separate IRS fax cover sheet template built for that purpose.

How to make a fax cover sheet template in Word

Building a fax cover sheet in Word takes about five minutes. Paste a template from above, convert it to a two-column table, and save it as a .dotx file. Here are the steps in order.

Step 1: Start a blank document. Open Word and choose Blank Document. Set the margins to Normal (1 inch). Do not use the built-in fax templates from the Microsoft gallery. Most of them carry heavy shading that prints poorly on the receiving end.
Step 2: Paste the template text. Copy one of the five templates and paste it with Keep Text Only. That strips any web formatting. Type the title in 20-point bold. Leave the rest at 11 or 12 points.
Step 3: Convert the fields to a table. Select the lines from "To:" through "Subject:". Go to Insert, then Table, then Convert Text to Table. Choose two columns and separate text at colons. Word puts the labels in the left column and the placeholders on the right.
Step 4: Hide the table borders. With the table selected, open Table Design and set Borders to No Border. Keep one horizontal line under the header row if you like. Thin lines fax fine. Thick or colored borders turn into black bars.
Step 5: Pick a fax-safe font. Use Arial or Calibri at 11 points or larger. Times New Roman also works. Fax resolution is low, around 200 dots per inch. Thin serif fonts and anything under 10 points break apart on the page.
Step 6: Save it as a Word template. Choose File, then Save As, and pick Word Template (.dotx). Name it "Fax cover sheet". Next time, File, then New, then Personal shows it as a starting point. Microsoft explains the process on its page about saving a document as a template.

One more tip. Add a Date field instead of typing the date. Insert, then Date and Time, then check Update Automatically. The cover sheet then shows the correct date each time you open the template.

How to build the same template in Google Docs

In Google Docs, the fastest route is Insert, then Table, then a 2 x 8 grid. You then type the labels by hand. Convert Text to Table does not exist in Docs, so the paste-and-convert trick from Word will not work.

Start with a blank document at docs.google.com. Type the title at the top and set it to Heading 1. Then insert the table. Put "To:", "Fax:", "From:", "Phone:", "Date:", "Pages:", and "Subject:" in the left column. Type the bracketed placeholders in the right column.

To hide the borders, click inside the table and open Format, then Table, then Table properties. Set the border width to 0 pt. Add a paragraph below the table for the message and one more for the confidentiality notice.

Google Docs has no personal template gallery on free accounts. The workaround is simple. Name the document "Fax cover sheet TEMPLATE" and make a copy each time with File, then Make a copy. Workspace accounts can submit the file to the organization's template gallery instead. Google documents that option on its help page for Docs templates.

How to save the cover sheet as a PDF and fax it

Save the finished cover sheet as a PDF and put it first in your document stack. Then upload the files to your fax service. PDF locks the layout, so the fax looks the same on any receiving machine.

In Word, choose File, then Save As, then PDF. In Google Docs, choose File, then Download, then PDF Document. Both exports keep the table layout and fonts intact.

Merging the cover with the rest of the pages is optional. Most online fax services accept several files and send them in the order you upload them. Put the cover sheet first. Then add the contract or the records behind it.

Common cover sheet mistakes that cause failed or misrouted faxes

Count the cover sheet as page one. If you fax a three-page contract, the total is four. Write "4" in the Pages field, not "3". The receiving fax machine prints a count of its own. A mismatch triggers a phone call or a discarded fax.

Do not use colored backgrounds or shaded table cells. Fax machines are black and white. Light gray shading comes through as speckled noise that hides text. White background, black text, nothing else.

Skip the fill-in-the-blank underscores. Many downloadable templates use lines like "To: __________". They look fine on paper but tempt you to write by hand. Handwriting at 200 dots per inch is hard to read. Type every field.

Keep the notice short. A confidentiality clause longer than four lines pushes the message off the page. Two or three sentences cover the point. The templates above already fit within that limit.

Avoid decorative fonts. Script and condensed typefaces lose their thin strokes during transmission. When you do not need Word or Google Docs at all

You can skip Word entirely when you only need a printable cover or when you fax from a phone. Two shortcuts cover most situations.
